I'm sorry for my long silence on this topic.  After a fair bit of 
discussion and consultation, I've settled on ?-> as the spelling I'd
like to see for your &&-> defined dereference.  

If you're up for submitting a revised patch, I'd like to see if we can
get it applied and tested more widely before the 5.13 release on the
20th.
