[PATCH] full API for cop hint hashes

Attached patch provides a full API for cop hint hashes, *without*
exposing the refcounted_he type per se.  The implementation is just
a macro layer, so there's no runtime cost, but the set of functions
supplied and their documentation have a different viewpoint from the
underlying refcounted_he API.
