[PATCH] tidy up DieNull and DIE_NULL

This patch allows Perl_die to have a NULL argument - corresponding to throwing an exception.

This allows various bits of code (previously complicated by format checkery) to be simplified.

The macros DieNull and DIE_NULL have been removed: they don't appear to used in CPAN modules.
