Ok, so this patch removes the case-insensitivity of /(not )?ok/ and
changes /skip(ped)?/i back to /skip/.  It also removes some extra
strictness I added to _parse_test_line() which got confused on test
comments.  Both restore Test::Harness's original logic.

I'm also adding a test, t/lib/sample_tests/with_comments (taken from
Test.pm's suite) to prevent this from happening again.  That and I'm
documenting the ability to add comments to your testing code by
placing them on seperate lines.  The documentation's view of reality
is a bit stricter than the code is (it claims you need a # before
comments), but that's a Good Thing.
