Have you looked into an IP telephony solution?  There are relatively cheap
Cisco gateway devices (for home use, even -- I think it's called Komodo
ATA186)  that you could do this with.  I'm not sure what kind of software
support there is for it, but as far as I know, it may use SIP, MGCP &
H.323 protocols.
