[perl #38647] [PATCH] Standardize formatting of PDDs

The attached suggested patch standardizes the format of the completed  
PDDs to what's defined in docs/pdds/pdd_template.pod. It doesn't  
alter any content in the PDDs.

It also adds two optional sections to docs/pdds/pdd_template.pod:  
DEFINITIONS and LANGUAGE NOTES. They first appeared in PDD21 and are  
useful enough to be relevant in many PDDs
