New DBD module: DBD::Simulated

Hi all,

I created a new DBD module and submitted it to CPAN.

DBD::Simulated is using the prefix "simulated_" and could create any DBI 
error code required for testing software based on DBI.

I don't know if it should be registered in DBI at all because it doesn't 
have any private methods.
